Supplemental Formulary
The Preferred Product Program allows for those fund beneficiaries covered under American Consolidated Natural Resources Plans, Crimson Oak Grove Resources Plans, and UMWA International Plan to obtain “preferred” medication products from select classes for the usual copayment. “Non-preferred” products may be obtained for an additional charge plus the copayment.
You can avoid the additional charge if you ask your doctor to change your medication from a non-preferred drug to one that is on the preferred drug list.
If you cannot take a preferred product due to a specific medical reason or if your doctor believes that you must have the non-preferred product, the doctor should fax a letter of medical necessity to 1-888-487-9257 to provide medical information as to why the preferred product should not be dispensed. If approved, you will be authorized to receive a “non-preferred” product at the standard copayment price.
